Registered patients are prioritised for chronic condition care planning under the new MBS framework starting 1 July 2025.

If you are an existing patient at Deer Park Medical Centre and wish to transfer your healthcare to a different GP clinic, please have the new clinic send us a fax or email request with the signed patient authority, including their preferred method of transmission and contact details.
Please note that Deer Park Medical Centre charges a fee of $65 for preparing and transferring patient's medical records. This fee covers various aspects, including the time needed to supervise and inspect the records, costs for assessing and collating your health information, making copies, and postage via Australia Post. Upon receiving the request, one of our staff members will contact you to arrange the fee and initiate the process.
Once payment is received, please allow 7 to 10 business days for us to reconcile the payment, compile the information, and send it to your new clinic.
Alternatively, if you prefer, we can provide a health summary at no cost. This summary will include key information from your records to help assist your management at the new medical centre.
