MyMedicare Registration

My Medicare is a voluntary patient registration initiative introduced by the Australian Government to strengthen the relationship between patients, their general practice, and preferred GP.  It is part of a broader strategy to improve continuity of care, especially for people with chronic or complex conditions.

What is MyMedicare?

MyMedicare:

• Allows patients to formally register with their regular general practice and preferred GP.

• Supports better coordinated and continuous care.

• Enables access to new Medicare benefits and funding models tied to long-term care planning.

• Encourages patients to develop a consistent care team, which is particularly valuable for managing chronic conditions, mental health, or frequent medical needs.

Why Should Patients Register at Deer Park Medicare Centre?

Registering at Deer Park Medicare Centre provides several patient benefits:

1. Priority Access to Services
   Registered patients are prioritised for chronic condition care planning under the new MBS framework from July 2025.

2. Continuity of Care
   Patients receive more consistent care from a familiar team that understands their medical history and needs. Providing support to their health and wellness journey.

3. Access to New MBS Items
   Since July 2025, patients must be registered through My Medicare to be eligible for new GP Chronic Condition Management Plans and review items at their nominated practice.

4. Better Coordination with Allied Health
   Registration allows GPs at Deer Park to coordinate referrals and care pathways more effectively with allied health providers.

5. No Cost to Register
   Registration is free and can be done easily online or at the clinic.

Transfer of Medical Records Form

Are you a new patient wanting to transfer your medical records from your previous GP or medical centre?

If you are a new patient and would like to transfer your previous medical records to Deer Park Medical Centre, click on the link: TRANSFER OF MEDICAL HISTORY REQUEST, fill in your details and fill in your details prior to your appointment.

Are you an existing patient wanting to transfer your medical records from Deer Park Medical Centre to your new GP medical centre?

If you are an existing patient at Deer Park Medical Centre and wish to transfer your healthcare records to a different medical clinic, it is recommended to have the new clinic send a fax or email request with your signed consent, providing authority for your records to be released. Ensure the preferred method of transmission and contact details are included. 

Deer Park Medical Centre has a fee of $45 for preparing and transferring patient's medical records. This fee covers the time needed to review your records, assess, collate and compile the information and organising distribution to the requested clinic.

Upon receiving the request, one of our friendly team members will contact you to initiate the process and process your payment. Once completed, allow 10 business days for the provision of the request.

Should you only require a summary, of your history, including results, plesase ensure this is mentioned on the requested form. Please note that there is no charge for the health summary.
